ANNUAL GENERAL MEETING (AGM) OF MEMBERS
OF THE CANADIAN FEDERATION OF MENTAL HEALTH NURSES (“CFMHN”)

The Board of Directors (the “Board”) wants your voice to be heard at this year’s AGM. Also, in order for any votes to take place a quorum is required.
For any members who are unable to participate in person, the CFMHN bylaws allow for voting by proxy. We encourage anyone not able to attend in person to submit a proxy vote. Click here for the proxy form.
If you do not know any other CFMHN members, you may wish to provide your proxy to:
- CFMHN president Sarah Flogen (Ottawa)
- CFMHN treasurer Stephen VanSlyke (Fredericton)
- your provincial CFMHN representative (you can find yours on the list linked to here: https://www.cfmhn.ca/board-of-directors/.
For the purpose of ensuring that all proxy voting is subsequently verifiable, you must use our proxy vote form. Please email this proxy form on or before Oct. 31, 2022 at 4:00 p.m. Proxy vote forms will not be accepted after Oct. 31 - not even on the day of the AGM.
This proxy vote form is only valid for the 2022 CFMHN AGM on Nov. 7, 2022 or on a subsequent date if the AGM is adjourned to another date.
