Dear Wound Care Nurses Interest Group members,
We are excited to present an opportunity to share your expertise to review the resource:
Population Health Primary Care Pathway to Delay Diabetic Foot Ulcer Onset and Progression (Draft).
This pathway is being developed collaboratively by RNAO and Wounds Canada as a knowledge translation tool to support primary care providers in delivering evidence-informed, consistent, and practical care to reduce the risk and progression of diabetic foot ulcers at a population level.
This work is being undertaken in two phases:
- Phase 1: Development of the population health pathway (current draft for review), providing a high-level overview
- Phase 2: Development of a detailed implementation toolkit to support uptake and application of the pathway in practice, where we will seek your consultation
At this stage, we are asking members of the Wound Care Nurses Interest Group to review the Phase 1 draft pathway and provide feedback on its clinical accuracy, clarity and any gaps or areas that may require further detail.
It should take about 10-15 minutes to complete
