We are seeking the feedback and expertise of the Primary Care Nurses of Ontario Interest Group to review the Population Health Primary Care Pathway to Delay Diabetic Foot Ulcer Onset and Progression draft. This pathway is being developed collaboratively by RNAO and Wounds Canada as a knowledge translation tool to support primary care providers in delivering evidence-informed, consistent, and practical care to reduce the risk and progression of diabetic foot ulcers at a population level.
This work is being undertaken in two phases:
- Phase 1: Development of the population health pathway (current draft for review), providing a high-level overview
- Phase 2: Development of a detailed implementation toolkit to support uptake and application of the pathway in practice, where we will seek your consultation
At this stage, we are asking members of the Primary Care Nurses of Ontario Interest Group to review the Phase 1 draft pathway and provide feedback on its clinical accuracy, clarity and any gaps or areas that may require further detail.
The draft pathway serves as a high-level summary and will be complemented by a comprehensive implementation toolkit that will provide more detailed guidance for each stage. Development of the toolkit is anticipated to take approximately six to eight weeks and will include a small series of working meetings, followed by structured review and refinement cycles.
