    1. Motion: Be it Resolved that the name of the Ontario Family Practice Nurses be changed to the Primary Care Nurses of Ontario.
  4. Finance Director Report
    1. Motion: Be It Resolved that the name of our general bank account be changed from Ontario Family Practice Nurse Group to Primary Care Nurses of Ontario
    2. Motion: Be It Resolved that the name of our education fund be changed from the Terry O’Neill Fund OFPN to The Terry O’ Neill Fund P.C.N.O.
    3. Motion: Be It Resolved that the name of our Paypal account be changed from Ontario Family Practice Nurses to Primary Care Nurses of Ontario
    4. Motion: Be It Resolved that the membership fee continues as $50 for general members, $30 for new grads and retirees both of which include membership in Canadian Family Practice Nurses Association(CFPNA), and free for students which does not include membership in CFPNA.
  5. Bylaw Change: Be it Resolved that all references to Ontario Family Practice Nurses (OFPN) in the Bylaws be changed to Primary Care Nurses of Ontario (PCNO)

CALL FOR NOMINATIONS for EXECUTIVE POSITIONS

This is a Call for Nominations for the Executive of the Ontario Family Practice Nurses.  Nominations are being accepted for President, Vice President, Communications Director, Finance Director, and Member-at-Large.  Nominees must indicate their acceptance of the Nomination either verbally at the meeting or in writing to the President.

 

CALL FOR NOMINATIONS for TERRY O’NEILL AWARD

THE TERRY O’NEILL AWARD

The OFPN Terry O’Neill Award recognizes nurses who have made a contribution to the Ontario Family Practice Nurses and to the profession of nursing in Ontario in the area of primary care nursing. The award is presented every two years in conjunction with the OFPN conference.


The Criteria used to determine the recipient for the Award includes:

  • Outstanding contributions to the Ontario Family Practice Nurses
  • Member of nursing profession
  • Recognized as a role model and mentor for peers and/or students
  • Individual contributions in the clinical practice area of primary health care.


Criteria for nominations:

  • Nominee and nominator must be current members of the OFPN and have been members for at least one year
  • Nominations may come from a Chapter Group President on behalf of that Chapter or from individual members

Procedure for ALL nominations:

All nominations must be accompanied by the following:

  • Nominee’s resume or summary
  • A letter of support for the nomination (no longer than 5 pages) based on the criteria for selection as outlined
  • Each nomination requires two signatures
  • All nominations are to be sent to info@ofpn.ca
